Ngala is a private hospital and requires each patient admitted to the hospital to have a GP or medical practitioner to authorise each admission.
If you are not the medical practitioner, please print the completed form, insert your details in the appropriate section and return it to the family for their doctor to complete and sign.
Request for admission forms need to be signed by your family GP or medical practitioner.
